
Overview
Enhanced care management (ECM) is a Medi-Cal benefit in California available to eligible youth through their managed care plans (MCPs). ECM provides intensive, person-centered care management, addressing both medical and social needs.

Get Started
To be eligible for ECM, youth up to age 21 must be enrolled in a Medi-Cal managed care plan and meet one or more of the following criteria:
- Experiencing homelessness
- Transitioning out of incarceration
- Current or former foster youth (up to age 26)
- Significant behavioral health needs
- Experiencing frequent hospitalization
Contact your Medi-Cal managed care plan to be guided through the referral and enrollment process. Referrals can come from any individual member, medical provider, or community support.
