Enhanced Care Management

Overview

Enhanced care management (ECM) is a Medi-Cal benefit in California available to eligible youth through their managed care plans (MCPs). ECM provides intensive, person-centered care management, addressing both medical and social needs.

Approach

A Seneca ECM Lead Care Manager works with enrolled members to assess their needs and create an individualized care plan to help them reach their wellness goals. The healthcare manager then gets them access to the care they need. This can include engaging with care teams such as doctors and psychiatrists; finding and scheduling medical appointments; arranging transportation to and from appointments; and identifying additional social services resources in the community that they may be eligible for and helping them enroll (like housing, food, and other benefits).

Seneca is contracted with 10 managed care plans to provide ECM to young people and serves nearly 2,000 beneficiaries annually. Youth enrolled in ECM are connected to ongoing services to meet their individual needs. This helps prevent the escalation or worsening of symptoms by ensuring they receive prompt and quality care.

To be eligible for ECM, youth up to age 21 must be enrolled in a Medi-Cal managed care plan and meet one or more of the following criteria:

Experiencing homelessness
Transitioning out of incarceration
Current or former foster youth (up to age 26)
Significant behavioral health needs
Experiencing frequent hospitalization

Contact your Medi-Cal managed care plan to be guided through the referral and enrollment process. Referrals can come from any individual member, medical provider, or community support.
